How to use this template

    Name the segment and the direction

    Record the access point you started from, the one you were heading to, and which way you ran. Published municipal CCTV specifications call for the run to be made upstream to downstream wherever possible, one access point to the next. Zero the counter at the start so every distance on the report is measured from the same place.

    Log each observation at its footage

    Distance on the counter, what you are looking at, and where it sits on the clock face — 12 o’clock is the crown, 6 the invert. Use the second clock box where the condition wraps. Add the photo or still reference while you are at that footage, not afterwards.

    Grade it, and pick the axis

    Grade 1 to 5, where 5 is the most severe. Then say whether it is structural — the pipe wall, the joints, the alignment — or operation and maintenance, which is anything fouling the bore that cleaning would move. Each observation type starts on the axis it usually belongs to and you can switch any row. A sound service connection is a feature, not a defect: log it with no grade at all.

    Read the two condition blocks, not one number

    The report gives you a peak grade, occurrences at that peak, a total score and a mean — computed twice, once for structural and once for O&M. They are never averaged together. A grade 5 structural break and a clean maintenance record is a very different line from a grade 3 on both.

    Choose the recommendation and export

    Pick the action yourself. The builder will not invent one from the grades, because no published specification sets a general grade-to-action threshold — agencies weigh severity against extent, criticality and consequence of failure. Fill in your PACP certification number if you hold one; leave it blank and every export says plainly that this is not a certified PACP deliverable.

What a sewer camera report has to get right

Almost every free sewer camera inspection report you can download quotes a "grade" without ever saying whose scale it came from. The grades on a sewer report come from the NASSCO Pipeline Assessment Certification Program, and the scale runs 1 to 5 with 5 as the most severe. That direction is the part people invert. A homeowner who reads "grade 5" on a report and hears a school grade concludes the line is in excellent condition, when in fact it is the worst thing the operator saw all day. This builder prints the direction of the scale on the document itself, next to the grades, in every export.

The second thing the copies flatten is that a sewer condition is not one number. NASSCO scores defects on two separate axes — structural, meaning the pipe wall, the joints and the alignment, and operation & maintenance, meaning roots, deposits and water getting in — and reports the ratings for those axes separately. A line can be structurally excellent and completely blocked with grease, or perfectly clear and about to collapse. Averaging those two into a single "condition grade" destroys the only information that tells you whether the answer is a jetter or an excavator. This report keeps them apart and publishes no merged figure at all.

The segment-level numbers here are derived from your own observations the way the published convention describes: a peak grade paired with how often that peak occurs, a total score that rises with both how bad the defects are and how many there are, and a mean. Nine grade-1 observations and a single grade-5 can produce a similar total, which is exactly why the peak and the count are printed alongside it rather than the total on its own.

Then there is the certification question, which the free templates ignore entirely. PACP is a certification program: certification is valid for three years from issuance and has to be renewed, and municipal CCTV specifications routinely require the coding to be done to PACP by a certified coder. A report that looks like a PACP deliverable but was not produced under a current certification is a problem for the contractor who signed it. This builder records the operator’s certification number, and when that field is empty every export states in plain words that the document is a contractor’s condition report and not a certified PACP deliverable.

One thing this builder deliberately refuses to do is turn a grade into a recommendation on its own. There is no published national specification that says a grade 4 means lining and a grade 5 means replacement — that mapping is local capital-planning policy, weighed against how much of the line is affected, how critical the line is, and what failure would cost. Inventing a threshold would put a wrong verdict on somebody’s job, so you choose the action and the report prints your grades beside it. Where you recommend cured-in-place lining, the report cites ASTM F1216, the standard practice that governs it.

Finally, it says what was not inspected. If the camera covered 60 feet of an 85 foot segment because it hit a blockage, the report says so and names the 25 feet that were never televised. Untelevised footage is precisely where a reader assumes nothing was found, and a report that quietly omits it is the one that comes back as a dispute.

Is grade 1 or grade 5 worse on a sewer inspection report?

Grade 5 is worse. On the NASSCO PACP scale, condition grades run from 1 to 5 and 5 is the most severe — a pipe that has failed or is about to. Grade 1 is the least severe, minor deterioration consistent with normal ageing. This is the reverse of a school grade, and it is the most commonly inverted number in the whole trade. This report prints the direction of the scale on the document so nobody has to remember it.

Structural defects are defects of the pipe itself — cracks, fractures, offset joints, broken or collapsed sections. They drive repair, lining and replacement decisions. Operation and maintenance conditions foul or obstruct the bore without breaking the pipe — roots, grease, deposits, groundwater getting in — and they drive cleaning. NASSCO grades and reports the two separately, and this builder never merges them into a single number, because the two conditions call for completely different work.

Not on its own. Only a currently PACP-certified operator can issue a coded PACP report, and PACP certification is valid for three years from issuance. This builder is a contractor’s condition report that references the PACP 1–5 severity scale. There is a field for the operator’s PACP certification number: leave it blank and every PDF, Excel and Word export states in plain words that the document is not a certified PACP deliverable.

No, and deliberately. The PACP defect-code tables, the code letters and the grade definitions are NASSCO’s licensed, copyrighted material and are not reproduced here. What this report implements is the publicly documented framework only — the 1 to 5 scale with 5 most severe, the separate structural and O&M scoring, and segment figures derived from the individual observation grades — described in plain English, with NASSCO cited as the source of the scale.

On a clock face, looking in the direction you are travelling: 12 o’clock is the crown of the pipe and 6 o’clock is the invert. Municipal CCTV specifications require the clock position of every defect and every connection to be recorded against the footage counter. This builder gives you a "from" and a "to" position, so a crack running from 10 to 2 records as the arc it actually is rather than as a single point.

Published municipal CCTV specifications call for the inspection to be made in the upstream to downstream direction wherever possible, one line section at a time between access points, with the footage counter zeroed at the start of each run. The direction matters on the report as much as in the field: the same defect sits at a completely different footage measured from the other end, so this report records which way you ran alongside every distance.

It will not decide that for you, and any template that claims to should be treated with suspicion. There is no published national specification that maps a grade to an action — that mapping is a local capital-planning policy that weighs severity against extent, how critical the line is, and the consequence of failure. You choose the recommendation from the list and the report prints your grades next to it, so the reader can see both the evidence and the judgement.

Record the footage you actually televised alongside the segment length and the report calls it out: it names how many feet were not inspected and states that no condition is reported for them. That untelevised run is exactly where a reader assumes nothing was found, so a report that quietly leaves it out is the one that comes back as a dispute later.
