How to use this template

What is a Plumbing leave request form?

It is a simple form a plumbing employee uses to request time off — vacation, sick, personal, or other leave — and for a manager to approve or deny it in writing. This builder totals the days requested, tracks leave balances, and produces a signed PDF, Excel, or Word record for both parties.

The form counts working days between the start and end dates, excluding Saturdays and Sundays, since that is what draws down most PTO balances. It also shows the total calendar days for reference. For a partial day, switch to half-day or hourly mode.

Nine common types: vacation/PTO, sick leave, personal days, bereavement, jury duty, parental or family leave, unpaid leave, floating holidays, and an "other" catch-all. Pick the one that fits, and use the reason field for anything specific.
