How to use this template

  1. 1

Fill in employee and employer details

Enter the electrical employee's name, ID, title, department, and their manager, plus your company details. The request number and submission date are pre-filled.

  2. 2

    Choose leave type and dates

Pick the leave type — vacation, sick, personal, bereavement, and more — and whether it is full days, a half day, or a set number of hours. Set the dates and the form counts the working days automatically, excluding weekends.

  3. 3

Add coverage and, if you track it, balances

Note who covers the work, how to reach the employee if needed, and any handover details. Turn on the leave-balance table to show accrued, used, and remaining days so the approver has the full picture.

  4. 4

Get the manager decision and sign-off

The manager marks the request approved, denied, or pending with an optional note. Capture both signatures and export a clean PDF, Excel, or Word file for the employee and the personnel file.

Why electrical contractors need a standard leave request form

On an electrical crew, one electrician out can stall a job that needs a licensed hand on site. That makes it worth knowing about time off early and in writing — with the dates, the coverage plan, and a clear approval on record.

This form removes the guesswork. Choose the leave type and dates and it counts the working days, tracks accrued and remaining balances if you use them, and records the manager decision with both signatures — clean documentation for payroll and the personnel file.

Use this free electrical leave request form to standardize time off across your team. Pair it with CloseCrew so Larry, your AI receptionist, keeps the phones covered and jobs scheduled while an electrician is out.

Leave Request Form FAQs

What is a Electrical leave request form?

It is a simple form a electrical employee uses to request time off — vacation, sick, personal, or other leave — and for a manager to approve or deny it in writing. This builder totals the days requested, tracks leave balances, and produces a signed PDF, Excel, or Word record for both parties.

The form counts working days between the start and end dates, excluding Saturdays and Sundays, since that is what draws down most PTO balances. It also shows the total calendar days for reference. For a partial day, switch to half-day or hourly mode.

Nine common types: vacation/PTO, sick leave, personal days, bereavement, jury duty, parental or family leave, unpaid leave, floating holidays, and an "other" catch-all. Pick the one that fits, and use the reason field for anything specific.

Optionally, yes. Turn on the leave-balance table and enter accrued and used days for each leave type — the form calculates the remaining balance automatically, so the approver can see whether the employee has the time available before signing off.

The employee signs to make the request, and the manager or approver signs to record the approve/deny decision. Both signatures (typed or uploaded) appear on the exported form, giving you a clear, dated record for the personnel file.
