How to use this template

    Set up the certificate

    Add the certificate number and date, then the permit number and the authority having jurisdiction. Referencing the permit is what ties this record to the inspection that actually approves the installation.

    Describe the extent of the work

    Pick the type of work and describe precisely what the certificate covers — and what it does not. A record that says only “electrical work” cannot tell a future reader which parts of the installation were yours.

    Record the service as installed

    Enter the service type, size, main OCPD, AIC rating and available fault current, then the grounding electrode arrangement. If a single rod was used, add its measured resistance and the sheet will tell you whether a supplemental electrode is required.

    Set the test conditions

    The insulation test voltage and acceptance minimum default from the ANSI/NETA table row for your service. Override the minimum where the equipment manufacturer publishes a different figure — NETA’s own rule puts manufacturer data first.

    Add each circuit tested, record the insulation reading, and tap through continuity, polarity and AFCI. Enter GFCI trip current and time where a GFCI is fitted. Each row grades itself, then export to PDF, Excel or Word.

What a US installation certificate can and cannot say

It is worth being straight about what this document is, because the answer differs by country and the difference matters. In the UK, an electrician issues a BS 7671 Electrical Installation Certificate and personally certifies that the installation complies. In the United States nothing works that way. The forms a US contractor actually signs — the permit applications published by state and city authorities — are attestations that the application information is true, not that the finished work is compliant. The compliance judgement belongs to the inspector, and the certificate or sign-off is released by the authority having jurisdiction after a passed final inspection.

So this builder produces the document that genuinely exists in US practice and that clients keep asking for: the contractor’s own record of what was installed and what was tested. It captures the service characteristics, the grounding electrode arrangement, and a per-circuit schedule of test results, and it states plainly on every export that it does not replace the AHJ’s inspection or approval. That framing is not a disclaimer bolted on at the end — it is the reason the document is trustworthy. A sheet that implied it were an approval would be worse than useless to the person holding it.

The numbers behind the grading come from the standards that US practice actually uses. Insulation resistance is judged against ANSI/NETA ATS Table 100.1, whose own footnote describes its values as suggested representative figures rather than pass-fail limits — which is why the acceptance minimum here is a default you can edit rather than a constraint. GFCI results are judged against UL 943 Class A: a Class A device trips at 6 mA and above and must not trip below 4 mA, with a maximum trip time following an inverse-time curve. And the grounding electrode section applies NEC 250.53(A)(2) as what it actually is — a decision about whether a single rod needs a second one, not a verdict on the installation.

Is there an official US electrical installation certificate?

Not one issued by the contractor. In US practice the contractor files a permit application, an inspector performs the inspection, and the authority having jurisdiction releases the certificate or sign-off after a passed final. That pattern holds across every state and city form we examined. This template is the contractor’s own completion and test record — the document you give the client alongside the AHJ’s approval, not instead of it.

A BS 7671 Electrical Installation Certificate is contractor-self-certified and carries UK-specific fields — TN-S, TN-C-S and TT earthing arrangements, Ze and prospective fault current, ring final continuity, and a competent-person scheme registration number. None of those apply in the US, and the acceptance values differ sharply in both directions: a GFCI trips at 4–6 mA where an RCD is rated at 30 mA, while the NETA insulation minimum at 600 V is 100 MΩ against BS 7671’s 1 MΩ. This template uses US terms and US thresholds throughout.

The builder defaults to ANSI/NETA ATS Table 100.1: 25 MΩ at a 250 V rating tested at 500 V DC, and 100 MΩ at a 600 V rating tested at 1000 V DC, applied for one minute. NETA’s own note calls these suggested representative values, and its actual rule is that the equipment manufacturer’s published data takes precedence where it exists. The minimum is therefore editable rather than fixed.

This is the most commonly misread rule in the Code. NEC 250.53(A)(2) does not set a 25-ohm pass-fail limit for the grounding electrode system. It says that a single rod, pipe or plate electrode measuring 25 ohms or less does not require a supplemental electrode. Above 25 ohms, a second electrode is required. Once two electrodes are installed and bonded, the requirement is satisfied by arrangement and no resistance measurement is required at all. The builder models it that way — as a decision about the supplement, never as a verdict on the installation.

Record the residual current at which the device tripped, and the trip time if your tester reports it. A UL 943 Class A device must trip at 6 mA or above and must not trip below 4 mA — a device tripping at 3 mA fails just as surely as one that holds until 8 mA. Where a trip time is entered, it is checked against the UL 943 inverse-time curve at the measured current.

Anything installed that differs from the code edition you cited, along with the reason. Leaving it blank reads as “none” on the finished certificate, which is the correct default for most jobs. Recording a departure honestly at handover is far cheaper than having it discovered later with no explanation attached.

It suits commissioning and close-out on commercial work, and the acceptance values follow the NETA specification that owner specs commonly invoke. For full NETA acceptance testing on large equipment, the owner’s specification usually prescribes its own report format and tighter thresholds than the table defaults — record those in the editable minimum field and attach this as the circuit-level record.
